Provide case management services to members in the Two Rivers region facing complex medical conditions.
Develop and implement individualized care plans for members, in collaboration with healthcare providers and other stakeholders.
Conduct initial and ongoing assessments of member needs and identify potential barriers to care.
Coordinate and communicate with healthcare providers, insurance companies, and other relevant parties to ensure efficient and effective care for members.
Monitor and evaluate member progress and adjust care plans as needed.
Advocate for members and assist in navigating the healthcare system.
Maintain accurate and detailed case notes and documentation.
Collaborate with interdisciplinary teams to coordinate services and resources for members.
Educate members on their healthcare options and promote self-management strategies.
Stay up-to-date on industry developments, regulations, and best practices related to case management.
Adhere to company policies and procedures, as well as relevant laws and ethical standards.
Participate in training and professional development opportunities to continuously improve skills and knowledge.
Represent Aetna in a professional and compassionate manner at all times.
Travel to meet with members and attend meetings and trainings as needed.
