Role Description
The Director, CHW Operations will join Zócalo Health during a period of rapid growth and increasing operational complexity. This role exists to lead and scale the Community Health Worker function within Zócalo's RN-led care management model.
As part of a fast-growing team, you will be responsible for building high-performing field operations, developing Community Health Worker leaders, driving consistent execution of community-based interventions, and ensuring CHWs are effectively integrated into multidisciplinary care teams.
This position reports to the SVP, Care Delivery.
The Director, CHW Operations will contribute in the following ways:
Lead all Community Health Worker operations across markets, including workforce planning, deployment, productivity, and performance management
Establish consistent standards for home visits, community-based interventions, member engagement, outreach, and field documentation
Build management systems that support coaching, accountability, quality, and professional development across the CHW organization
Partner closely with Nurse Practitioners, RN Care Managers, Behavioral Health Case Managers, and Care Coordinators to ensure seamless integration within care teams
Monitor operational performance and identify opportunities to improve member engagement, retention, and outcomes
Support expansion into new markets while maintaining consistency of service delivery and operational excellence
Serve as the primary leader responsible for the effectiveness and scalability of the CHW function
Qualifications
8+ years of healthcare operations leadership experience
Experience leading Community Health Workers, care coordinators, patient navigators, outreach teams, or other community-based healthcare workforces
Experience building and managing distributed field teams
Strong understanding of Medicaid populations and social determinants of health
Demonstrated success developing frontline leaders and improving operational performance
Excellent communication, coaching, and change management skills
Preferred Qualifications
Experience with ECM, Community Supports, CalAIM, or other Medicaid care management programs
Experience working with culturally diverse and historically underserved populations
Multi-state operational experience
Experience scaling community-based healthcare teams during periods of growth
Bilingual Spanish/English strongly preferred
