Job Summary
As a Care Team Associate (CTA) - Remote, you will provide critical support to our care management team, ensuring the successful implementation and execution of all Care Management processes. This is a full-time position that requires flexibility to work any shift between 8:30 AM and 5:00 PM, Monday through Friday. As a CTA, you will serve as a liaison with internal and external customers, providing excellent customer service and support to ensure optimal care coordination and health outcomes.
Key Responsibilities
- Provide clerical and administrative support to clinical staff and managers for special projects and reporting needs
- Document all member interactions in a concise manner, compliant with documentation requirements for Model of Care, NCQA, and CMS regulations
- Receive and respond to incoming Care Coordination inquiries from various communication venues, including phone, email, and electronic platforms
- Coordinate and assist in monitoring documentation Care Management queues for Disease Management, Transitions of Care, Complex Care, and Social Work referrals
- Process CM requests via all communication venues and prepare administrative materials for clinical staff
- Conduct inbound and outbound calls for program requirements, including member scheduling, surveys, and satisfaction/complaints
- Complete timely data entry of member contact information into software applications
- Provide excellent customer service to internal and external customers, serving as a resource and support
- Attend required meetings and participate in ad hoc committees as needed
- Maintain knowledge of health plan benefits, network, NCQA regulations, and health plan policies
- Identify opportunities for process improvement in all aspects of member care
- Communicate effectively to improve patient health literacy and maintain strict confidentiality at all times
Requirements and Qualifications
Essential Qualifications:
- High School Diploma or GED required
- Must be 18 years of age or older
- Ability to prioritize, plan, and handle multiple tasks/demands simultaneously
- Advanced knowledge of Microsoft Office products, including Microsoft Word, Excel, and Outlook
- Ability to work any shift between 8:30 AM and 5:00 PM, Monday through Friday
Preferred Qualifications:
- Certified Medical Assistant training or certification
- 2+ years of experience in a physician's clinic or hospital
- 2+ years of administrative support experience
- Additional experience working in a medical care setting as a receptionist or medical assistant
- Medical terminology knowledge base
- Excellent organizational skills, customer service skills, and verbal and written communication
- Ability to work independently with some supervision and direction from manager
Telecommuting Requirements:
- Reside within commutable distance to the office at 3 Farm Glen, Farmington, CT
- Ability to keep all company-sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service
Physical and Work Environment:
- Ability to lift up to 25 pounds
- Ability to sit for extended periods
- Ability to stand for extended periods
- Ability to use fine motor skills to operate office equipment and/or machinery
- Ability to receive and comprehend instructions verbally and/or in writing
- Ability to use logical reasoning for simple and complex problem-solving
Compensation, Perks, and Benefits:
We offer a competitive salary range of $16.00 to $28.27 per hour, depending on experience and qualifications. In addition to your salary, you will have access to a comprehensive benefits package, including:
