Job Summary
We are seeking an experienced Data Entry Specialist to join our Patient Care Coordinator team. As a Patient Care Coordinator, you will be responsible for handling monthly refill requests, ensuring accurate documentation, and coordinating with patients, families, and healthcare professionals to provide exceptional patient care. Key Responsibilities
As a Patient Care Coordinator, you will be responsible for:
- Answering incoming calls from patients, families, and healthcare professionals regarding nutrition treatment requests and status updates.
- Triage calls to different departments as needed.

Required Qualifications

- Experience working in a home infusion or durable medical equipment (DME) setting.
