Key Responsibilities
- Verify Plan of Care: Collaborate with the RN to verify the plan of care prior to each visit, ensuring that patient needs are met and care is coordinated effectively.
- Patient Care and Education: Teach patients and caregivers self-care techniques, check vital signs, review medications, and perform medication reconciliation at each visit.
- Specialized Procedures: Perform specialized procedures as ordered by the physician, such as foley catheter changes, using aseptic technique to prepare equipment and materials for treatments.
- Communication and Collaboration: Collaborate frequently with physicians and RNs to ensure patient needs are being met, notifying them promptly of any changes in patient condition.
- Documentation: Complete documentation accurately and in a timely manner, maintaining high standards of record-keeping.
Qualifications and Requirements
Essential Qualifications:
- One year of clinical LPN experience, demonstrating a strong foundation in patient care and nursing skills.
- Current LPN license in the state(s) where you will be working, ensuring compliance with regulatory requirements.
- Current CPR certification, highlighting your commitment to maintaining up-to-date life-saving skills.
- Valid driver's license, reliable transportation, and liability insurance, facilitating travel to patient homes.

Compensation, Perks, and Benefits

- A competitive salary range of $24.00 to $27.00 per hour.
