About the Role
This is a fully remote opportunity, allowing you to work from anywhere in the United States, as long as you meet the required licensure and work hour requirements. As a Case Manager RN, you will be responsible for telephonically and/or in-person assessing, planning, executing, and coordinating all case management activities with members to evaluate their clinical needs and facilitate overall well-being.
Key Responsibilities
- Collaboratively develop a proactive plan to resolve issues presented to enhance short and long-term outcomes, as well as identify opportunities to improve a member's overall health through integration.
- Conduct assessments of members' needs and available benefits using clinical tools and data/information review (e.g., assessments, claims, etc.) to collaborate and refer to programs offered by the plan sponsor.
- Apply clinical judgment to reduce risk factors, address complex medical issues, and social determinants.
- Utilize case management processes in compliance with regulatory and organizational policies and procedures.
- Employ strong communication and interpersonal skills to ensure maximum member engagement and identify their health status and health needs based on key questions and conversation.
Essential Qualifications
To be successful in this role, you must possess:
- A current and unrestricted RN license in your state of residence. If located within the Nurse Licensure Compact (NLC) region, the ability to utilize your compact license without restriction is required.
- Willingness and ability to obtain additional state licenses upon hire (paid for by the organization).
- 3+ years of acute care clinical experience as an RN (general medical, post-surgical, specialty including pediatrics, ICU, case management, and discharge planning).
- Ability to use a computer station with multiple screens, work multiple projects simultaneously, and sit for extended periods.
- A dedicated workspace free from distractions and fast internet connectivity.
Preferred Qualifications
To stand out as a strong candidate, you should have:
- Compact RN licensure.
- Certified Case Manager (CCM) certification.
- Strong client support skills, including attention to clients, prevention of specific issues, and proactive identification/resolution of issues.
- Experience with a variety of Microsoft Office applications, including PowerPoint, Excel, and Word.
- Strong telephonic communication skills.
- 3+ years of experience using computers and navigating multiple screens while speaking with someone on the phone.
