About the position Responsibilities • Conduct comprehensive assessments of patient and family needs. • Establish and modify the plan of care based on needs assessment. • Supervise the progress of care and ensure modifications are made as needed. • Incorporate evidence-based nursing practices into care plans. • Collaborate with patients and families to set realistic and measurable outcomes. • Coordinate and delegate care to ensure safe and effective patient care delivery. • Fulfill all organizational requirements and responsibilities. Requirements • Associate's or Vocational degree in Nursing. • 0 to 2 years of RN experience. • Licensed Registered Nurse in Minnesota required upon hire. • BLS Tier 1 - Basic Life Support certification required by completion of orientation. • Valid Driver's License required upon hire. Nice-to-haves • Bachelor's degree in Nursing. • 0 to 2 years of homecare/hospice experience. • CHPN Certified Hospice and Palliative Nursing certification preferred. • RN-BC RN Case Management certification preferred. • ANCC-RNCM Certification in Case Management preferred.
