Job Overview
We are seeking a dedicated and compassionate Nurse or Social Worker - Intake Coordinator to join our healthcare team. This role is crucial in ensuring that new patients receive a thorough assessment and seamless transition into our care facility. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to providing high-quality patient care. The position is Full Time working 8:00PM - 8:30AM Remote (3 12's) and includes every other Saturday.
Responsibilities
• Conduct comprehensive patient assessments during the admissions process, including medical history and physical. .
• Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient's needs.
• Ensuring adherence to safety protocols and best practices.
• Provide education to patients and families regarding treatment plans, medication administration, and health management strategies.
• Maintain compliance with HIPAA regulations while handling sensitive patient information.
• Participate in discharge planning and follow-up care coordination for patients transitioning out of the facility.
• Engage in ongoing professional development to stay current with best practices in nursing care.
Qualifications
• Current Registered Nurse (RN), LMSW licensure in the state of practice.
• Experience in acute care settings, psychiatric hospital, or long-term care preferred.
• Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation practices.
• Strong assessment skills with the ability to recognize changes in patient conditions promptly.
• Previous experience working with diverse populations, including geriatrics.
• Knowledge of pain management techniques, infection control practices, and behavioral health considerations is beneficial.
