Responsibilities
- Assist patients during admissions, transfers, and discharges, ensuring their personal care needs are met in accordance with their wishes and care plans.
- Review care plans daily to determine if changes have been made to the patient's daily care routine.
Qualifications
- CNA license certification prior to hiring.
- Current CPR and fingerprint clearance card.
- Minimum of six (6) months CNA/LNA experience preferred, with recent Long-Term Care/Skilled Nursing Facility or Transitional Rehab experience.
