Lead Nurse Care Manager - Madbury

Responsibilities:

1. Manage and coordinate the care of residents utilizing the nursing process.
2. Collaborate with team members to formulate individualized resident care plans.
3. Assess and document changes in residents' physical or mental status and report findings to the primary physician and treatment team.
4. Provide leadership and direction for direct care partners.
5. Establish relationships with team members, residents, and family members to facilitate open, direct communication.

Requirements:

• Nursing degree or diploma from an accredited college, university, or nursing school.
• Long-term care facility experience and specialized knowledge of nursing care of the elderly and rehabilitative nursing preferred.
• Nursing license within the State of New Hampshire.
• Resident Assessment Tool certification.
