Job Overview We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to join our innovative Vitals and Remote Patient Monitoring (RPM)/Chronic Care Management (CCM) Program. In this vital role, you will be responsible for monitoring patient vital signs, managing remote health data, and supporting individuals with chronic conditions through comprehensive patient assessment and care coordination. Your expertise will help improve patient outcomes by providing timely, accurate health assessments and ensuring adherence to long-term care plans. Duties • Conduct routine vital sign assessments including blood pressure, pulse, respiration, temperature, and oxygen saturation for patients enrolled in the RPM/CCM program. • Utilize electronic health record (EHR) systems such as Epic or Athenahealth to document patient data accurately and efficiently. • Monitor remote patient data streams, identify abnormal trends, and escalate concerns to the healthcare team promptly. • Perform triage by assessing patient symptoms, providing health coaching, and guiding patients on medication adherence or symptom management. • Support patients with complex needs such as Alzheimer's care, dementia management, or chronic pain by delivering personalized education and emotional support. • Assist with medication administration, IV infusion procedures, and catheter care when necessary within scope of practice. • Collaborate with multidisciplinary teams including physicians, case managers, and specialists to coordinate comprehensive patient care plans while adhering to HIPAA and confidentiality policies. Skills • Proven experience with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and data management. • Critical care experience including ICU or emergency room settings is highly desirable for handling acute situations effectively. • Strong knowledge of vital signs assessment, physiology principles, and medical terminology to interpret clinical data accurately. • Experience working with diverse populations such as geriatric patients, individuals with developmental disabilities, or those requiring long-term care services. • Skilled in triage protocols, infection control practices, and basic life support (BLS) certification to ensure patient safety at all times. • Familiarity with ICD coding (ICD-9/ICD-10), medical documentation standards, and diagnostic evaluation techniques. • Excellent communication skills to educate patients about their health status and foster a supportive environment for behavioral health or memory care needs.
