LPN, Skilled Home Visits Fulltime

Conduct home visits to provide skilled nursing care to patients in their own homes. Assess patient needs and develop personalized care plans. Administer medications and treatments as prescribed by physicians. Monitor patients' health status and report any changes or concerns to the healthcare team. Educate patients and caregivers on disease management, medication administration, and other healthcare topics. Collaborate with other healthcare professionals to coordinate and improve patient care. Maintain accurate and up-to-date patient records. Adhere to all regulatory and legal requirements related to home healthcare. Provide emotional support and build trusting relationships with patients and their families. Continuously seek opportunities to improve patient outcomes and satisfaction. Attend and participate in team meetings and training sessions. Stay updated on industry developments and best practices in home healthcare. Comply with company policies and procedures.
