Responsibilities:
As a Medical Social Worker with CenterWell Home Health, your primary responsibilities will include:
- Assessing patients' social and emotional needs, and developing a personalized plan of care to address these needs
- Evaluating patients' relationships with their medical and nursing needs, financial resources, and community resources
- Providing direction and supervision to Social Worker Assistants as required
- Collaborating with the interdisciplinary care team to develop and implement patient care plans
- Participating in discharge planning and identifying community resources to support patients and their families
- Maintaining accurate and timely documentation, including case conferences, patient/physician contacts, and visit reports
- Supervising, instructing, and evaluating the performance of Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards
Requirements:
To be successful in this role, you'll need:
Essential Qualifications:
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice (required by state law or regulation)
- A valid driver's license, auto insurance, and reliable transportation
- Proof of current CPR certification
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
Preferred Qualifications:
- Experience with discharge planning and community resource development
- Knowledge of medications and their correct administration
- Excellent oral and written communication and interpersonal skills
- Ability to organize tasks, develop action plans, set priorities, and function under stressful situations
- Ability to be flexible in work hours and travel locally
