Physical Therapist (PT) & Physical Therapist Assistant (PTA)
Full-Time | Part Time | Home Health | Northeast Florida
We’re seeking compassionate, motivated Physical Therapists and Physical Therapist Assistants to join our growing rehabilitation team.
You’ll provide one-on-one care in patients’ homes, helping them improve mobility, reduce pain, regain independence, and safely remain in the place they call home.
What You’ll Do
• Evaluate patients and develop individualized treatment plans (PT)
• Provide skilled physical therapy interventions based on physician orders
• Educate patients and caregivers on exercises, mobility, fall prevention, and safety
• Collaborate closely with nurses, physicians, occupational therapists, speech therapists, and medical social workers
• Document visits accurately using our streamlined electronic documentation systems with AI-assisted support
• Monitor patient progress and modify treatment plans as appropriate
• Help patients achieve meaningful functional goals and improve quality of life.
What We’re Looking For
• Current Florida PT or PTA license
• Home Health experience preferred, but motivated clinicians with strong clinical skills are encouraged to apply
• Excellent clinical judgment and communication skills
• Ability to work independently while remaining connected with a highly supportive interdisciplinary team
• Strong organizational and documentation skills
• Passion for delivering compassionate, patient-centered care
