Responsibilities
• Provide the delivery of culturally sensitive, age-specific team-based care models in delivering nursing care for Primary Care patients including adult medicine, pediatric, and OB/GYN populations.
• Perform phone triage, including assessment, screening, clinical advice, nurse visits (in person/video), in-clinic hands-on procedures, and other duties assigned.
• Accurate and efficient documentation of nursing care.
• Knowledge and appropriate use of internal and external resources.
• Provide functional supervision to assistive personnel; collaborate with colleagues, other disciplines, and health care/community services to promote continuity of care.

• Knowledge and skills in patient and family education and coaching for chronic conditions (e.g., diabetes, asthma, CAD).
• Proficiency with typing and electronic health record documentation.
• Ability to manage complex clinical issues with assessment, protocols, and evidence-based interventions.
• Willingness to work with different populations (pediatrics, OB/GYN, adult medicine); orientation/training for new subspecialties may be provided.
Qualifications
• Three (3) years RN experience in Adult Internal Medicine (inpatient, emergency, or ambulatory)
• One (1) year OB/Gyn RN experience in Ambulatory Care, Labor and Delivery

Hours/Week: 40 hours
Shift/Hours/Days:
Day Shift;
Mon-Fri, includes Saturdays (variable hours)
Location:
East Ann Arbor Health Center with potential to work in other locations as division expands
