Job Summary:
- Integrate healthcare services for patients from pre-admission to post-discharge, ensuring coordinated and sequenced care.
- Evaluate patient outcomes based on clinical, financial, and utilization data, utilizing independent nursing judgment and age-specific care methodologies.
- Communicate effectively with patients, families, and healthcare teams, facilitating groups and implementing projects as needed.
- Demonstrate fiscal responsibility, controlling variances and duplication, and documenting clearly and concisely.
- Work autonomously, prioritizing tasks, and adapting to changing situations, with strong flexibility and organizational skills.
Requirements:
- Bachelor's degree in nursing preferred, with a minimum of 1-2 years of professional nursing experience in a clinic or home health setting.
- Current RN license with the State Board of Nursing, with multistate licensure preferred if in a Nurse Licensure Compact (NLC) state.
- Department-specific competencies and certifications, with a valid driver's license required depending on location.
