Job Overview:
We're seeking a compassionate and experienced Registered Nurse to join our team as a Case Manager, working remotely from North Carolina. As a key member of our Aetna One Advocacy team, you'll play a vital role in delivering personalized, high-touch care to our members, empowering them to take control of their health and wellbeing.
About the Role:
In this dynamic and rewarding position, you'll utilize your clinical expertise to assess, plan, implement, and coordinate comprehensive care plans for our members. Through telephonic and face-to-face interactions, you'll evaluate medical needs, facilitate wellness initiatives, and provide ongoing support to enhance short-term and long-term outcomes. Your expertise will be instrumental in reducing risk factors, addressing complex health and social indicators, and promoting overall wellness.
Key Responsibilities:
- Develop proactive care plans to address member needs and enhance overall wellness
- Conduct comprehensive assessments, considering multiple diagnoses, co-morbidities, and functional limitations
- Apply clinical judgment to incorporate strategies that reduce risk factors and barriers to care
- Collaborate with multidisciplinary teams to present cases and drive effective claim management
- Utilize case management processes in compliance with regulatory and company policies
Requirements:
- Current, unrestricted RN license in state of residence, with multi-state/compact privileges
- 3+ years of clinical practice experience as an RN
- Associates Degree in Nursing required; BSN preferred
- Willingness to obtain all 50 state licenses upon hire
- Residence within a commutable distance of High Point, North Carolina (typically within 45 minutes)
Preferred Qualifications:
- Case Management experience in an integrated model
- Certified Case Manager (CCM) certification
