Job title: Youth Transition Services Virtual Teacher Company: HUMANIM INC Job description: Program Coordination Assist in the recruitment and screening of youth for the transition services program, working collaboratively with school personnel and the Division of Rehabilitation Services (DORS) with both in-person and virtual services.Ensure that all youth participants receive high quality work experience and develop the necessary job readiness skills.Disseminate information to the program manager on an ongoing and continuous basis.Act as a liaison to partners by facilitating and sustaining positive and effective relationships.Monitor and track participant progress throughout the duration of involvement with the program.Coordinate all class schedules for 8-week classes year round.Communicate effectively with all county coordinators regarding students who are assigned per their regions.Maintain documentation and grading scheme for all students enrolled. Documentation Complete all required paperwork and documentation, both electronically and hardcopies, in an accurate and timely manner.Properly document and maintain progress/contacts notes regarding all communications and relevant activities associated with each participant in the program.Enter all data into the AWARDS & VirtualJobShadow.com database within 24-48 hours.
