Population Health Analyst

The Population Health Analyst supports the design, build, and maintenance of Epic Healthy Planet and Compass Rose tools to enable population health management, care coordination, and quality improvement programs. This role focuses on registry and measure development, clinical program logic, and dashboard configuration to support internal operations and value-based care initiatives. The analyst collaborates with application teams, care management, and operational leaders to ensure build aligns with clinical workflows and reporting needs. Rate- Hourly W2 up to $90/hr Key Responsibilities - Design, build, and maintain registries, measures, dashboards, and program logic in Epic Healthy Planet and Compass Rose. - Translate clinical and program requirements into Epic configuration specifications. - Support technical implementation of care management programs, quality tracking, and risk stratification tools. - Collaborate with application analysts, data/reporting teams, and clinical stakeholders to align build with operational goals. - Test, validate, and troubleshoot Epic functionality tied to population health and care coordination programs. Required: Epic Healthy Planet certification, Epic Compass Rose certification, 3 years of experience in Epic application build or clinical program support, experience configuring registries, measures, dashboards, and care programs in Epic, understanding of population health workflows and quality program requirements, strong analytical and problem-solving skills, attention to detail and commitment to data integrity, ability to work collaboratively across technical and clinical teams, bachelor’s degree in public health, health informatics, nursing, or a related field, familiarity with value-based care programs and clinical reporting is a plus Apply tot his job Apply tot his job
