Registered Nurse – Health Coach Consultant, Chronic Conditions

Job Description:• The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation• Engage, educate, and promote and influence member's decisions related to achieving and maintaining optimal health status• Assessment of Members through the use of clinical tools and information/data review• Conducts comprehensive evaluation of member's needs and benefit plan eligibility for available integrated internal and external programs/services• Utilizes assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change• Application and/or interpretation of applicable criteria and guidelines, health/wellness management plans, policies, procedures, regulatory standards• Consults with supervisors, Medical Directors and/or others to overcome barriers to meeting goals and objectives• Identifies and escalates quality of care issues through established channels• Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices• Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member's goals.Requirements:• A Registered Nurse that must hold an unrestricted license in their state of residence• Multi-state/compact privileges and have the ability to be licensed in all non-compact states• 3+ years of clinical experience in Adult Medical Surgical nursing• Must possess or be willing and able to obtain high speed internet access and be hard wired to internet• Proficiency with Microsoft Office application: Word, Outlook, Excel, Teams and Power Point• Experienced with documenting in an electronic medical records system• Excellent communication skills• Must be comfortable with public speaking• Ability to work independently as well as part of a groupBenefits:• Affordable medical plan options• 401(k) plan (including matching company contributions)• Employee stock purchase plan• No-cost programs for all colleagues including wellness screenings• Tobacco cessation and weight management programs• Confidential counseling• Financial coaching• Paid time off• Flexible work schedules• Family leave• Dependent care resources• Colleague assistance programs• Tuition assistance• Retiree medical access Apply tot his job
