RN or LPN –Remote Reportables/Investigations Nurse (Regional)Location:Remote (with limited travel as needed)Position Type: Full-TimeDescriptionWe are seeking an experienced RN or LPN to oversee reportables and incident investigations across multiple skilled nursing facilities. This position is remote and focuses heavily on regulatory compliance, documentation accuracy, and timely reporting. Key Responsibilities• Manage and complete state and federal reportables accurately and on time• Conduct, coordinate, or support investigations of incidents, injuries, and allegations• Review facility documentation for completeness and compliance• Work with facility teams to clarify facts and gather follow-up information• Assist with root cause analysis and help identify preventable factors• Maintain detailed logs, tracking, and ongoing follow-up on open investigations• Communicate updates and expectations to facility leadership and regional team• Ensure confidentiality and integrity during all investigative processesQualifications• Active RN or LPN license (multistate preferred)• Experience in skilled nursing or long-term care required• Strong understanding of reportables, regulatory requirements, and incident procedures• Excellent documentation, communication, and critical-thinking skills• High attention to detail and ability to work independentlyPreferred Experience• Prior work with investigations, risk management, or compliance• Experience preparing survey-related documentation or POCs• Knowledge of Iowa and/or Kansas regulations would be ideal.Apply tot his job
