Utilize your strong clinical background to assess and evaluate members' medical needs and develop individualized care plans. Collaborate with healthcare providers and other members of the medical team to ensure coordinated and comprehensive care. Serve as the primary point of contact for members and their families, providing support, guidance and education throughout their healthcare journey.Monitor and track members' progress and outcomes, making necessary adjustments to care plans as needed. Advocate for members and help them navigate the healthcare system to access the resources and services they need. Maintain accurate and detailed documentation of all interactions and interventions with members. Stay up-to-date on industry trends and best practices in case management to continuously improve the quality of care provided. Demonstrate compassion, empathy and understanding towards members and their families during difficult and challenging situations.Collaborate with other members of the bolthires team to ensure a seamless and positive experience for members. bolthires is an Equal Opportunity Employer.
