**Experienced Full Stack Social Worker – Remote Medical Case Management**

As an experienced full stack social worker at arenaflex, you'll have the opportunity to partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. You'll be responsible for researching available options and resources for members with financial, transportation, safety, and housing needs, and working to empower members to make positive lifestyle and medical choices. **Job Summary** We're seeking an experienced full stack social worker to join our team in a remote work-from-home role. As a licensed Masters Social Worker, you'll be responsible for providing medical case management services to members across the entire lifecycle. You'll work closely with a registered nurse case manager to assess, plan, and advocate for members' needs, and provide support and resources to help them achieve their health and wellness goals. **Responsibilities** * Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Work to empower members to make positive lifestyle and medical choices * Provide support and resources to help members achieve their health and wellness goals * Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Conduct telephonic or electronic exchanges with members to assess their needs and provide support * Develop and implement individualized plans to meet members' needs and goals * Monitor and evaluate the effectiveness of plans and make adjustments as needed * Maintain accurate and up-to-date records of member interactions and progress **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence * 2+ years of experience in medical social work or case management * Masters degree in social work or a related field * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills * Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities * Strong computer skills and experience with electronic medical records and other healthcare software **Preferred Qualifications** * Experience working with diverse populations and in a healthcare setting * Knowledge of community resources and services for medical, financial, and behavioral health * Experience with case management software and other healthcare technology * Strong analytical and problem-solving skills * Ability to work in a remote environment and maintain confidentiality and security of member information **Benefits** At arenaflex, we're committed to providing our employees with a comprehensive benefits package that includes: * Competitive salary and bonus structure * Comprehensive medical, dental, and vision benefits * 401(k) retirement savings plan with company match * Employee Stock Purchase Plan * Fully-paid term life insurance plan * Short-term and long-term disability benefits * Paid Time Off (PTO) or vacation pay * Paid holidays throughout the calendar year * CVS store discount and discount programs with participating partners * Free development courses and education assistance * Well-being programs and resources to support your physical and mental health **Work Environment** As a remote full stack social worker at arenaflex, you'll have the flexibility to work from home and maintain a healthy work-life balance.
