This full-time position offers a unique opportunity to work with disengaged, vulnerable members who often face multiple medical, behavioral, and social challenges. As a Case Manager RN, you will be responsible for assessing, planning, implementing, and coordinating all case management activities to evaluate the medical needs of members and facilitate their overall wellness.Key Responsibilities:• Conduct comprehensive assessments to evaluate members' needs and benefits, using clinical tools and information/data review. • Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers, addressing complex health and social indicators that impact care planning. • Develop proactive courses of action to address issues presented, enhancing short and long-term outcomes and opportunities for members' overall wellness through integration. • Collaborate with supervisors and other key stakeholders in members' healthcare to overcome barriers in meeting goals and objectives.• Utilize case management processes in compliance with regulatory and company policies and procedures. • Employ motivational interviewing skills to ensure maximum member engagement, discerning their health status and health needs based on key questions and conversations. • Work on behalf of the client and the member's provider, advocating for their needs and facilitating seamless care coordination. EssentialQualifications:To be successful in this role, you will need:• To be a Registered Nurse with an unrestricted nursing license and multi-state/compact privileges, with the ability to be licensed in all non-compact states.• 3+ years of clinical/nursing practice experience, with a strong foundation in diabetes management. • 1+ year of experience in care coordination or discharge planning, with a proven track record of success in telephonic case management. • High-speed internet access, with the ability to hardwire your computer, and proficiency in personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications. PreferredQualifications:While not required, the following qualifications are highly desirable:• Telephonic case management experience, with a strong understanding of remote patient engagement strategies.• Excellent communication and interpersonal skills, with the ability to build strong relationships with members, providers, and stakeholders. • An Associate's degree in nursing, with a BSN preferred, demonstrating a commitment to ongoing education and professional development. Skills and Competencies:To excel in this role, you will need to possess:• Strong clinical judgment and critical thinking skills, with the ability to analyze complex data and develop effective care plans. • Excellent communication and interpersonal skills, with the ability to engage members, providers, and stakeholders in care coordination efforts.• Proficiency in motivational interviewing, with the ability to empower members to take an active role in their healthcare. • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
