An outstanding employee health center is seeking a Nurse Practitioner for a hybrid telehealth role in East Stroudsburg, PA. JobResponsibilities: - APP's serve multiple serious conditions like CKD and/or ESRD/ESKD- Conducts assessments including annual wellness exams- Counsels and educates patients and their families about programs- Responsible for coordination of care with primary care providers- Part of the Integrated care team working with RN and Social Worker- Prescribe medications, order tests, collaborate with Monogram physician- Follow up and evaluate patient's statusQualifications: - NP, FNP, AGNP, ACAGNP with Active PA license- Board Certified- ANCC or AANP- 2 or more years of direct patient care- DEA, BLSWorking Hours: Mon-Fri (8a-5p includes travel time for in-home visits) Apply tot his job
