Our case managers meet with individuals in their homes, complete strength-based assessments and develop customized care plans focused on client preferences. Our goal is to improve or maintain the clients' quality of life, independence, and overall health and safety. Working in tandem with a nurse cars manager. our staff collaboratively maintains relationships, coordinates all functional and social supports that promote independence and maintain client safety, and creates a plan of care to improve overall well-being is the core of our program. This is a full-time, predominantly first shift position requiring a valid Wisconsin Driver's License, access to a vehicle, personal motor vehicle insurance (meeting minimum coverage limit requirements of the Agency), passing a Department of Justice Background Check and Federal Sex Offender Registry Check, and verification of references.
