The Role: LPN Home Health - Geriatric Care Specialist
As a LPN Home Health professional with Amedisys, you will work one-on-one with our geriatric population, providing essential care and support in their homes. This is a full-time position, with a coverage area in Nash County, North Carolina. Our administrative office is located in Franklinton, North Carolina, and we offer a competitive salary range of $25.00 to $30.00 per hour, reflecting your skills and experience.
Key Responsibilities:
- Verify plan of care with the RN prior to each visit, ensuring seamless coordination of care.
- Collaborate with physicians and RNs to ensure patient needs are being met, and notify them promptly of any changes in patient condition.
- Teach patients and caregivers self-care techniques, empowering them to take an active role in their care.
- Check vital signs, review medications, and perform medication reconciliation at each visit.
- Perform specialized procedures, such as foley catheter changes, as ordered by physicians.
- Prepare equipment and materials for treatments using aseptic technique, ensuring a safe and effective care environment.
- Complete documentation in a timely and accurate manner, maintaining comprehensive patient records.
Qualifications and Requirements:
To be successful in this role, you will need:
- One year of clinical LPN experience, with a strong foundation in geriatric care.
- A current LPN license, specific to the state(s) you are assigned to work.
- Current CPR certification, with a commitment to maintaining up-to-date certifications.
- A valid driver's license, reliable transportation, and liability insurance, as you will be traveling to patient homes.
Preferred Qualifications:
While not required, the following qualifications are preferred:
- Experience working with geriatric populations, with a deep understanding of their unique needs and challenges.
- Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
