SummaryThe Telehealth Physical Therapist provides high-quality, patient-centered physical therapy services to residents within a long-term care (LTC) facility using secure telehealth technology. This role supports onsite clinical teams by delivering virtual evaluations, treatment sessions, clinical recommendations, and interdisciplinary collaboration to improve resident mobility, safety, and functional independence.Essential Duties and Responsibilities· Supports and promotes the mission and values of Covenant Health Ministry. · Conduct virtual physical therapy evaluations for LTC residents and rehabilitation patients, including mobility assessments, strength testing, balance analysis, and functional screenings. · Develop individualized treatment plans tailored to the needs, goals, and medical complexities of long-term care residents and rehabilitation patients. · Provide synchronous (live video) treatment sessions, instructing residents and patients—and when appropriate, caregivers, nursing staff, or therapy assistants—on exercises, mobility strategies, fall-prevention techniques, and functional training.· Collaborate closely with onsite therapy teams (PTs, PTAs, OTs, COTAs, SLPs) and nursing staff to ensure coordinated and effective care delivery. · Communicate real-time recommendations to onsite staff regarding transfers, mobility equipment, positioning, precautions, and environmental modifications. · Provide oversight and direction to onsite therapy assistants as permitted by state practice acts and facility policies. · Educate residents/patients, caregivers, and facility staff on home exercise programs, safety strategies, and restorative nursing activities.· Monitor resident/patient progress and modify treatment plans based on clinical need and functional outcomes. · Document evaluations, daily notes, and progress updates in accordance with facility guidelines, regulatory requirements, and professional standards. · Ensure compliance with all telehealth rules, including HIPAA, secure communication protocols, and state licensure requirements. · Participate in care plan meetings, interdisciplinary rounds, and facility quality-improvement initiatives as needed.Job RequirementsJob Knowledge and Skills· Strong verbal, written, and presentation communication skills· Responsible, supportive team player· Experienced providing telehealth services and supervising others in a telehealth capacity· Strong interpersonal and customer service skills· Analytical, able to make sound decisions· Proficient in Microsoft Office, telehealth technologies, and secure video systems· Ability to work collaboratively in an interdisciplinary team· Excellent time management and documentation skills· Compassionate and resident-centered care approach·The ability to speak, read, write, and understand written instructions in English· Must have reliable internet and an appropriate ergonomic work environment by which to work· Must adhere to the Covenant Health Telecommute Policy and sign the Covenant Health Telecommute PolicyEducation and Experience· Masters or Doctorate required.· Active license as a PT in the applicable State of work. · Basic Life Support (BLS) certification is preferred. · Prior experience in a long-term care or skilled nursing facility preferred. An equivalent combination of education and experience that provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements. Working Conditions/Physical Demands· Must possess the physical and mental abilities to perform the tasks normally associated with this position that involve a combination of sitting, standing, walking, bending, stooping, twisting, and reaching.· Must be able to lift up to 50 pounds. Above 50 pounds requires a 2 person lift. · Lifting/carrying frequently up to 20 pounds. Some stress is related to a high level of responsibility for quality care.
