About the positionResponsibilities• Analyze data collected from the payers to determine which patients require outreach. • Outreach to patients based on their adherence rate status, reminding patients when the next refill is due to pick up. • Educate patients and their families about the importance of medication adherence, ways to increase adherence, and when to communicate with health care team. • Assess patient barriers with regards to medication usage and escalate these barriers to patient health care team in a timely fashion so changes can be made.• Knowledgeable of Medication Adherence projects and can help facilitate or operationalize initiatives (e.g., >90-Day supply, Mail-Order, Med Sync, etc.). • Manage and provide a comprehensive summary and log of all active Medication Adherence patients. • Act as a resource for Quality Improvement staff, providers, office staff and patients. • Assist with the evaluation and amendment of Medication Adherence Procedures, Policies, and Protocols. • Attend continuing education sessions and training relating to your job functions as required.• Perform additional duties/tasks as assigned. Requirements• Must be an active Registered Nurse License in the State of Indiana in good standing. • Certification in Case Management, preferred. • Current CPR certification must be maintained (BLS). • 2-4 years of experience working within the primary care or hospital setting as a Registered Nurse.
