Job Overview We are seeking a highly skilled and compassionate Licensed Social Worker to join our team in a remote, telephonic case management role. As a Licensed Social Worker at arenaflex, you will have the opportunity to work collaboratively with registered nurse case managers to assess, plan, and advocate for members with complex health needs. Your expertise will be instrumental in empowering members to make informed lifestyle and medical choices, and in connecting them with vital community resources that support their overall well-being. Key Responsibilities Conduct telephonic assessments to identify members' benefit plan and health needs, and develop personalized plans to address these needs Research and provide information on available community resources, including financial, transportation, safety, and housing services Collaborate with registered nurse case managers and other clinical staff to deliver comprehensive care coordination and support Empower members to take an active role in their health care, and provide guidance on positive lifestyle and medical choices Utilize electronic exchange and telephonic communication to engage with members and facilitate care planning Qualifications and Requirements To be successful in this role, you will need to possess an active and unrestricted Licensed Social Worker (LSW) or Licensed Masters Social Worker (LMSW) license in your state of residence. A minimum of 2 years of experience in medical social work or case management is also required. You will need to have a Master's Degree in Social Work and excellent communication and interpersonal skills. Compensation and Benefits The typical pay range for this role is $46,597.20 - $101,228.40 per year, depending on experience and location.
